I would look in your Step by Step textbook once again. Page 845 for your first question. I think you are referring to a tool used by coders. Information on the development of ICD-10-PCS starting on page 898 through 904.
We do not receive any ICD-10 textbooks there are only drafts available at this time as ICD-10 doesn't go into effect until Oct. 2014. You can access ICD-10 at www.icd10data.com it is what I prefer to use for ICD-10.
